Victorian Quarry Operator Fined After Truck Driver Killed

A quarry operator has been convicted and fined $230,000 following an incident that claimed the life of a subcontractor in 2016.

The man was killed when his dump truck rolled on a stockpile.

WorkSafe Victoria reported that the quarry operator was fined after being found guilty of contravening section 26 of the OHS Act due to failure to ensure the workplace was safe and without risks to health and safety.

The driver, a man in his sixties died when the dump truck he was driving flipped over the edge of a stockpile at the Plumpton quarry and slid down the other side. He was moving material up an earthen ramp to dump near the top of the stockpile at the time.

An investigation revealed the quarry operator should have completed a complete risk assessment and should have had a Safe Work Method Statement for the task.

WorkSafe reminded employers that it is unacceptable to put the safety of any worker or driver at risk by failing to identify hazards and failing to take action accordingly.
